ABSTRACT

ABSTRACT Objective: To compare clinical and radiographical pulpectomy outcomes in primary teeth filled with different pastes. Material and Methods: The sample included thirty-eight teeth indicated for pulpectomy due to irreversible pulp inflammation or necrosis from thirty patients (2 to 9 years old). The first appointment comprised chemomechanical preparation (2.5% sodium hypochlorite), smear layer removal (6% citric acid), intracanal dressing and temporary restoration. Seven days later, teeth were randomly assigned to filling with iodoform (IP) or calcium hydroxide with zinc oxide (CHZO) based pastes and temporarily restored. Final restoration (composite resin) occurred at the 3rd appointment. Data from baseline, 6 and 12 months were analysed using descriptive and inferential statistics (p≤0.05). Results: The overall frequency of success was 63.6% (n=21), with no significant difference between groups (IP=62.5% n=10; CHZO=64.7% n=11, p=0.59). Multiradicular teeth, overfilled canals and teeth whose coronal restoration have been lost were significantly associated with failure (p=0.01, p=0.04 and p<0.001, respectively). Conclusion: After 12 months, both pastes showed similar outcomes and can be used as good options for pulpectomies in primary teeth. Moreover, tooth location, extent of the root canal filling, and integrity of final restoration during the follow-up influenced the outcome of pulpectomies.

ABSTRACT Objective: To evaluate a group of Brazilian dentists on their knowledge of Molar Incisor Hypomineralization (MIH) and Hypomineralized Second Primary Molars (HSPM) related to clinical aspects, consequences, and diagnostic criteria. Material and Methods: In this cross-sectional, the participants were invited by e-mail and Whatsapp® to answer a questionnaire about their knowledge of hypomineralization enamel defects (MIH/HSPM) on the Google Forms® platform. The questionnaire comprised eight questions about personal data and multiple-choice questions about their knowledge concerning clinical aspects, diagnostic criteria of MIH/HSPM and differential diagnosis through clinical images. Chi-square test was applied with the significance level set at 5%. Results: Most participants (n = 492; 91.1%) reported having knowledge about MIH/HSPM. The general dentists gave more incorrect answers (n = 40; 65.6 %;) about dental tissues affected by MIH/HSPM. Overall, 83.3% of the dentists gave the correct answer to which dentitions are associated with this condition. In addition, most dentists presented knowledge about the consequences related to possible fractures (n= 487; 90.2%) and about an increased risk of caries (n= 479; 88.9%) in the affected teeth. Regarding the differential diagnosis performed through clinical images, most participants gave incorrect answers (p≤0.001). Conclusion: The participants presented knowledge about the dentition associated with this condition and possible consequences related to the teeth affected by MIH/HSPM; however, they showed difficulties concerning clinical diagnostic criteria.

ABSTRACT

O procedimento de pulpectomia em dentes decíduos consiste na remoção total do tecido pulpar com inflamação irreversível ou com necrose, preparo químicomecânico e obturação com material reabsorvível, com o intuito de manter o dente o maior tempo possível na cavidade bucal, devolvendo função e saúde para o paciente infantil. Existem vários protocolos descritos na literatura, no entanto, não há consenso entre eles e poucos possuem acesso livre. Sendo assim, observa-se carência de um guia clínico e prático para nortear o cirurgião-dentista na realização desse procedimento durante o atendimento odontopediátrico. Portanto, o intuito deste trabalho é apresentar aos clínicos um protocolo de pulpectomia com altas frequências de sucesso clínico e radiográfico, baseado em evidências científicas, desenvolvido e aplicado pelo Grupo de Pesquisa de Endodontia em Odontopediatria da Universidade Federal do Rio de Janeiro (GEDOPED-UFRJ). O presente protocolo contempla indicações, contraindicações, tática operatória, materiais necessários e orientações para acompanhamento. Com esta publicação de maneira gratuita, de acesso livre e no idioma dominante do país, espera-se ampliar o alcance de odontopediatras e clínicos gerais que atendem crianças a um tratamento eficaz.


Pulpectomy in primary teeth consists of the complete removal of the irreversibly affected pulp or necrotic pulp tissue, chemical-mechanical preparation of the root dentin and filling root canals with a resorbable material, to keep the tooth as long as possible in the oral cavity, restoring function and health. There are several pulpectomy protocols described in the literature, however, there is no consensus among them and only few of them are open access. Therefore, there is a lack of clinical and practical guides to the practitioner in performing this procedure during pediatric dental care. The aim of this article is to present a clinically and radiographically successful pulpectomy protocol, based on scientific evidence, developed, and applied by the Research Group on Endodontics in Pediatric Dentistry at the Universidade Federal do Rio de Janeiro. This protocol includes indications, contraindications, operative techniques, materials, and guidelines for follow-up appointments. With this open access publication in Portuguese, we hope to disseminate an effective treatment and expand the outreach of a pulpectomy technique protocol to pediatric dentists and clinicians who treat children.

Introduction: Studies of twins are important because environmental and genetic factors seem to be related to the phenotypic alterations. Objective: This paper presents a unique case of monozygotic twins with mirror image of a retained primary central incisor. Case report: Twin male brothers, 9-years-old, presented prolonged retention of the primary central upper incisor. The over-retained teeth in one twin were a mirror image of those in the other twin. The first twin presented a prolonged retention of the tooth 51 whereas the other twin presented a prolonged retention of tooth 61. After radiographic exams the over-retained teeth were extracted. Conclusion: Twins may show similarity in pattern of dental anomalies supporting the influence of genetic factors. In identical twins the location of diagnosed anomalies can be mirror imaged. This fact should lead the professional to examine the pair of twins in order to diagnose any dental anomaly that may be present.


Introdução: Estudos feitos em gêmeos são importantes porque fatores ambientais e genéticos parecem estar relacionados às alterações fenotípicas. Objetivo: Este artigo apresenta um caso raro de gêmeos monozigóticos apresentando imagem em espelho de retenção prolongada de incisivos centrais decíduos superiores homólogos. Relato do caso: Os irmãos gêmeos, com 9 anos de idade, não apresentavam história de trauma orofacial ou doença comum da infância. Após exames clínicos e radiográficos foram identificados a retenção do dente 51 no gêmeo 1 e do dente 61 no gêmeo 2. Em ambos os pacientes, os dentes 11 e o 21 estavam em erupção. O tratamento proposto foi a exodontia dos dentes decíduos com anestesia local e acompanhamento. Conclusão: Gêmeos podem apresentar semelhança no padrão de anomalias dentárias devido à influência de fatores genéticos. Adicionalmente, em gêmeos monozigóticos, a localização das anomalias diagnosticadas pode se apresentar invertidas ou imagem em espelho. Esse fato deve estimular o profissional a examinar o par de gêmeos para diagnosticar qualquer anomalia dentária que possa estar presente. O diagnóstico precoce e tratamento adequados devem ser realizados para evitar danos funcionais e estéticos em pacientes com retenções dentárias.

Introduction: Description of the bacterial community before and after chemomechanical preparation (CP) with the removal of a smear layer (SL) in pulpectomized primary teeth has been little reported. Objective: These case reports describe the presence of total microorganisms and Enterococcus faecalis in root canals of primary incisors before and after CP with SL removal. Case Reports: Microbiological samples were collected from the root canals of three children (3.66±0.58 years old) with necrosis (n=2) and irreversible pulpal inflammation (n=1) in maxillary primary incisors. After teeth isolation with a rubber dam and antisepsis of the operative field, the sample collections were performed with sterile absorbent paper cones before and after the CP, which included irrigation with 2.5% sodium hypochlorite followed by 6% citric acid to remove the SL. The collected samples were analysed immediately at the end of the clinical procedures. The plates were incubated anaerobically for 48 hours at 37°C. The results were expressed as colony forming units (CFU)/mL. Results: Two of the three teeth showed total microorganisms before the CP. One incisor had no microorganisms in the initial collection. No CFU was counted in the samples collected after CP. Moreover, E. faecalis was not observed any time, either before or after the CP. Conclusions: E. faecalis was not detected in any sample, yet two of the three root canals had microorganisms before CP. In cases where microorganisms were initially found, 100% elimination was observed after the applied protocol.


Introdução: A descrição da comunidade bacteriana antes e após o preparo químico-mecânico (PQM) com remoção da smear layer (SL) em dentes decíduos pulpectomizados tem sido pouco relatada. Objetivo: Esses relatos de casos descrevem a presença de microrganismos totais e Enterococcus faecalis em canais radiculares de incisivos decíduos antes e após PQM com remoção de SL. Relatos dos Casos: Amostras microbiológicas foram coletadas do canal radicular de três crianças (3,66 ± 0,58 anos) com necrose (n = 2) e inflamação pulpar irreversível (n= 1) em incisivos decíduos superiores. Após o isolamento dos dentes com dique de borracha e antissepsia do campo operatório, as coletas das amostras foram realizadas com cones de papel absorvente estéril antes e após o PQM, que incluiu irrigação com hipoclorito de sódio 2,5% seguido de ácido cítrico 6% para retirada do SL. As amostras coletadas foram analisadas imediatamente ao final dos procedimentos clínicos. As placas foram incubadas em anaerobiose durante 48 horas a 37°C. Os resultados foram expressos em unidades formadoras de colônias (UFC)/mL. Resultados: Dois dos três dentes apresentaram microrganismos totais antes do PQM. Um incisivo não apresentava microrganismos na coleta inicial. Nenhuma UFC foi contada nas amostras coletadas após o PQM. Além disso, o E. faecalis não foi observado nenhum momento, nem antes, nem depois do PQM. Conclusão: Não foi detectado E. faecalis em nenhuma amostra, porém dois dos três canais radiculares apresentavam microrganismos antes do PQM. Nos casos em que foram encontrados microrganismos inicialmente, observou-se 100% de eliminação após o protocolo aplicado.

Abstract Objective: To verify the concordance in the evaluation of the apical limit of obturation (ALO) in filled root canals of primary teeth between digital and visual methods. Material and Methods: Twenty periapical radiographs of endodontically treated primary teeth were digitalized and evaluated by an endodontics specialist (E1), a PhD pediatric dentist (E2), and a MSc general dentist (E3). Calibrated evaluators (Kappa = 1.00) analysed the images in a light-isolated environment two times (D1 and D2) with a one-week interval between evaluations. ALO scores were categorized as overfilled, flush-filled and underfilled. Results: The intra-rater reliability between methods was 0.82 (D1) and 0.75 (D2) for E1, 0.93 (D1 and D2) for E2, and 0.94 (D1 and D2) for E3. Inter-rater reliability ranged from 0.71 (E1 × E3) and 1.00 (E1 × E2) for the visual method to 0.76 (E1 × E3) and 0.88 (E1 × E2) for the digital method. Spearman correlation coefficients showed a similar ranking among the evaluators. There was greater disagreement among the underfilled and ideal scores. For all evaluators, the digital method favoured the identification of the ideal score. Conclusion: Both methods are suitable for the determination of the ALO of filled primary teeth and can be used in clinical practice.

Abstract Objective: To evaluate the scientific evidence concerning the behavior rating scales efficiency to identify behavioral changes in preschool children undergoing dental treatment, through a systematic review. Material and Methods: MEDLINE/PubMed, Scopus, Cochrane Library, Web of Science, BVS databases and grey literature were searched. Also, a hand search of the included studies reference lists was conducted. Studies that evaluated healthy preschoolers' behavior before and after invasive dental treatments to observe behavioral changes were included. Two independent reviewers selected studies, extracted data and analyzed the risk of bias with a tool for before-and-after studies. The certainty of the evidence was evaluated with the GRADE approach. Results: Three studies were included. The Frankl Scale and North Carolina Behavior Scale were used in these studies. Both scales were able to identify behavioral changes in preschool children undergoing a dental intervention, although two of these included studies were considered fair with a high risk of bias, and one considered good with a low risk of bias. Conclusion: Although Frankl and North Carolina behavior scales were able to identify changes in the children`s behavior during dental treatment, these findings are not supported by strong evidence. Thus, further well-designed studies are needed to confirm this evidence.

Objective: This study aimed to associate parenting styles (democratic,authoritarian, and permissive) with the behaviour, and prevalence of caries lesionsamong preschool children submitted to dental care. Methods: At the initialconsultation, preschool children (n = 67), from two to six years of age, wereevaluated for behaviour through the Frankl scale. The parenting styles wereinvestigated through the Parenting Styles and Dimensions Questionnaire - ReducedVersion (PSDQ) and the prevalence of caries lesions through the dmft index.Sociodemographic and economic data were collected. The chi-squared test wasused for association among parenting styles, dmft index, type of behaviour(dichotomised as positive and negative), and the following independent variables:socioeconomic level, single child, school attendance, and educational level of theperson in charge. ANOVA followed by the Tukey test were used to compare themean dmft and parenting styles. Results: The majority of preschoolers presentedpositive behaviour (83.6%), and the dmft mean was 4.76 (± 3.43). Of the total,49.3% were democratic, 44.8% were permissive, and 6% were authoritarian. Therewas no association between parenting styles and all variables investigated (p >0.05). Conclusion: Considering the results, it can be observed that there was noassociation among parenting styles, caries prevalence, and behaviour of thepreschool children in an initial dental consultation.


Objetivo: Objetivou-se associar os estilos parentais (democrático, autoritário epermissivo) com o comportamento e a prevalência de lesões de cárie de pré-escolaressubmetidos ao atendimento odontológico. Métodos: Em consulta inicial, pré-escolares(n = 67), de 2 a 6 anos de idade, foram avaliados quanto ao comportamento, atravésda escala de Frankl. Os estilos parentais de seus responsáveis foram averiguadosatravés do Questionário de Estilos e Dimensões Parentais ­ Versão Reduzida (PSDQ)e a prevalência de lesões de cárie através do índice ceod. Foram coletados dadossociodemográficos e econômicos. Utilizou-se o teste do Qui-quadrado para associaçãoentre os estilos parentais, o índice ceod, tipo de comportamento (dicotomizado empositivo e negativo) e as variáveis independentes: nível socioeconômico, ser filhoúnico, frequentar escola e nível educacional do responsável. ANOVA seguido de Tukeyfoi utilizado para comparar as médias ceod e os estilos parentais. Resultados: Amaioria dos pré-escolares apresentaram comportamento positivo (83,6%) e a médiado ceo-d da população estudada foi 4,76 (± 3,43). Do total dos responsáveis, 49,3%eram democráticos, 44,8% permissivos e 6% autoritários. Não houve associaçãoentre os estilos parentais e todas as variáveis investigadas (p > 0,05). Conclusão:Diante dos resultados, pode-se observar que não houve associação entre os estilosparentais avaliados, prevalência de cárie e comportamento dos pré-escolares emconsulta odontológica inicial.

Abstract This study evaluated clinical and radiographic twelve-month outcomes of root canal treatments (CT) with smear layer removal, performed in primary teeth, using two different root canal filling materials. Pulpectomy was performed on 27 primary teeth with necrosis or irreversible pulpitis, caused by dental caries or trauma, in 23 children (2-7 years old). A single trained operator performed the CT in a single visit in cases without periapical or interradicular radiolucency (PIR) or in multiple visits in cases with PIR. Participants were selected based on specific inclusion and exclusion criteria, and randomly allocated into two groups: Group 1 (G1) - iodoform paste (iodoform + camphorated parachlorophenol + ointment comprising prednisolone acetate 5.0 mg and rifamycin 1.5 mg); Group 2 (G2) - Calen®/ZO paste. Treated teeth were restored with composite resin immediately after the root canal filling. The outcomes were evaluated clinically and radiographically according to specific criteria. Two blinded and standardized evaluators assessed the radiographic outcomes. We used descriptive analyses due to the small sample size. CTs were performed due to caries lesions in 70.4% of the cases and due to trauma in 29.6%. Only one tooth of G1 was unsuccessful; hence, pulpectomy performance in both groups was not influenced by the filling material, nor by any other analyzed variable. The level of the root canal filling was better in the Calen®/ZO group. The clinical and radiographic twelve-month outcomes indicated successful treatment, independently of the root filling material used.

Introduction: There is no consensus on the literature about the best root canal filling material for primary teeth. Objective: The aim of this study was to describe the root canal filling materials most widely used in clinical pediatric dentistry, showing the observed success rates. Sources of data: A search, limited to the last 10 years and, no language restriction, was performed in October 2018 in the PubMed, Cochrane and LILACS databases using the terms "pulpectomy" and "primary teeth". This narrative literature review included clinical and randomized controlled trials conducted on children who had undergone pulpectomy as root canal therapy, with at least six months of clinical and radiographic follow-up. Synthesis of data: A total of 254 papers were retrieved from the databases. After applying the inclusion and exclusion criteria, 25 papers were included in this review. Zinc oxide and eugenol (ZOE) based paste was still the most widely used root canal filling material for primary teeth in pediatric clinical research, showing an overall clinical and radiographic success ranging from 63.3% to 93.3%. Conclusion: ZOE based paste is still the most used material for root canal filling in primary teeth, although there is a trend in the use of iodoform with calcium hydroxide pastes, which show high frequencies of success.


Introdução: Não há consenso na literatura quanto ao material obturador ideal para canais radiculares de dentes decíduos. Objetivo : O objetivo deste estudo foi descrever os materiais obturadores para canais radiculares mais utilizados na clínica Odontopediátrica, mostrando os índices de sucesso observados. Fonte dos dados : Uma busca, limitada aos últimos 10 anos e sem restrição de idioma, foi realizada em outubro de 2018 nas bases de dados PubMed, Cochrane e LILACS usando os termos "pulpectomy" e "primary teeth". Esta revisão narrativa da literatura incluiu estudos clínicos controlados randomizados em crianças submetidas a pulpectomia como tratamento endodôntico, com, no mínimo, seis meses de acompanhamento clínico e radiográfico. Síntese dos dados : Foram recuperados nas bases de dados 254 artigos. Após a aplicação dos critérios de inclusão e exclusão um total de 25 artigos foram incluídos nesta revisão. A pasta à base de óxido de zinco e eugenol (OZE) foi o material obturador mais utilizado para canais radiculares de dentes decíduos em pesquisa clínica Odontopediátrica, mostrando sucesso clínico e radiográfico geral que variou de 63,3% a 93,3%. Conclusão : A pasta à base de óxido de zinco e eugenol é o material mais utilizado para a obturação de canais radiculares de dentes decíduos, embora exista uma tendência para a utilização de pastas à base de iodofórmio com hidróxido de cálcio, que apresentam altas frequências de sucesso.

Introduction: Adolescents present a potential risk for the development of caries and periodontal diseases. Objective: The objective of the present study was to review the indicators of self-care and oral health in brazilian adolescents. Methodology: The search strategy (adolescent AND oral health) AND Brazil* was used in Pubmed and LILACS databases. Articles published between 2012 and 2017 that presented data on self-care and oral health of brazilian adolescents were included in this review. Synthesis of data: 837 and 1375 articles were retrieved from the Pubmed and LILACS databases, respectively. After applying the inclusion and exclusion criteria, 7 articles were included. The characteristics of the studies and the data obtained regarding sel-care and oral health of the adolescents were presented descriptively. Most of the studies included adolescents between the ages of 15 and 19 years, the majority of whom sought dental care in less than one year of the interview and caries was the most prevalent disease compared to gingival bleeding. Conclusion: According to the indicators of self-care evaluated, the majority of Brazilian adolescents reported having sought dental care in less than one year and caries disease was the most prevalent condition.


Introdução: Adolescentes apresentam potencial risco para o desenvolvimento das doenças cárie e periodontal. Objetivo: O objetivo do presente estudo foi revisar, na literatura, dados acerca da saúde oral de adolescentes brasileiros através da prevalência ou presença de cárie e sangramento gengival, como indicadores indiretos (consequências), e frequências de escovação dentária, uso de fio dental e visita odontológica como indicadores diretos deautocuidado oral. Fontes dos dados: Utilizou-se a estratégia de busca ((adolescent AND oralhealth) AND Brazil*) nas bases de dados PubMed e LILACS. Os artigos publicados entre setembro 2012-2017 que apresentaram dados de ao menos um dos indicadores de saúde oral e autocuidado em adolescentes brasileiros, foram incluídos nesta revisão. As características dos estudos e os dados obtidos foram apresentados de forma descritiva. Síntese dos dados: Foram recuperados nas bases de dados PubMed e LILACS 837 e 1375 artigos, respectivamente. Após a aplicação dos critérios de inclusão e exclusão, 10 artigos foram incluídos. A maioria dos estudos incluiu adolescentes entre 15 e 19 anos, com relato de atendimento odontológico realizado em menos de 1 ano antes da entrevista, sendo a cárie a alteração bucal mais prevalente. Conclusão: Dentro das limitações do presente estudo, de acordo com os dados obtidos através dos indicadores de autocuidado pesquisados, os adolescentes brasileiros mostraram cárie como o agravo observado mais prevalente; as frequências de escovação e uso de fio dental variaram na região Sul; e relataram atendimento odontológico no período inferior a um ano da entrevista.

ABSTRACT

This study aimed to identify E. faecalis in saliva of patients that were divided into two groups: 10 patients with caries lesions and 10 caries-free patients. Material and methods: Saliva samples were collected with a sterile swab and inoculated in enterococcosel media for 48 hours. The positive samples were subcultured in broth agar - blood medium for storage and subsequent PCR analysis. The data were analyzed using the SPSS (÷2). Results: From the 20 cases included in the study, 3 were positive in the Enterococcosel medium, and both tests (culture and PCR) used confirmed that three of them belonged to the species E. faecalis. No samples were positive in Enterococcosel broth in the group of caries-free patients. From the three samples that were identified as Enterococcus in broth (positive bile esculin test), an amplified for E. faecalis PCR analysis (p > 0.005). Conclusion: Through the use of PCR, it was possible to identify the genus Enterococcus and the species E. faecalis in saliva of patients with carious lesions – the pathogen that may influence the prognosis of diseases of the oral cavity...


Este estudo objetivou identificar E. faecalis em saliva de pacientes que foram divididos em dois grupos: 10 pacientes com lesões de cárie e 10 livres de cárie. Material e métodos: Amostras de saliva foram coletadas com um swab estéril e inoculadas em meios Enterococcosel por 48 horas. As amostras positivas foram repicadas em meio de caldo de agar - sangue para armazenamento e análise de PCR subsequente. Os dados foram analisados utilizando o SPSS. Resultados: Dos 20 casos incluídos no estudo, três foram positivos no médio Enterococcosel e ambos os testes (cultura e PCR) utilizados confirmaram que três deles pertenciam à espécie E. faecalis. Nenhuma amostra foi positiva em caldo Enterococcosel no grupo de pacientes livres de cáries. A partir das três amostras que foram identificadas como Enterococcus em caldo (teste positivo esculina biliar), foram amplificados por análise de PCR para o E. faecalis (p>0.005). ABSTRACT

A terapia endodôntica nos dentes decíduos foi analisada por meio de 3.358 prontuários de crianças atendidas na Clínica de Odontopediatria da Faculdade de Odontologia da Universidade Federal do Rio de Janeiro (UFRJ), no período de 1990 a 2010. Os dados deste estudo retrospectivo foram tabulados no programa SPSS 16.0, e o protocolo da conduta clínica foi analisado por década (PI = 1990-2000; P2 = 2001-2010) utilizando-se o teste x(2). A média de idade das crianças foi de 4,5 ± 1,4 anos para a primeira década e de 5,5 ± 2,1 anos para a segunda década. Um dos principais motivos para o tratamento continua sendo a presença de cárie extensa (79,9%). Para a obturação dos condutos, no primeiro período, a pasta mais utilizada foi a proposta por Guedes-Pinto (76,5%), e no segundo, a pasta de óxido de zinco e eugenol (54,0%). Concluiu-se que não houve mudança significativa no protocolo das pulpectomias realizadas na Disciplina de Odontopediatria da Faculdade de Odontologia da UFRJ, embora tenham se observado pequenas variações nas técnicas utilizadas em relação ao material obturador

ABSTRACT

Objetivo: Avaliar a eficácia de pulpectomias com remoção de smear layer em dentes decíduos anteriores e as condições clínicas dos sucessores permanentes após cinco anos de acompanhamento clínico e radiográfico.Método: Foram selecionadas crianças com idade entre três a cinco anos, clinicamente saudáveis, com necessidade de tratamento do canal radicular em dentes anteriores decorrente de cárie e/ou traumatismo dento-alveolar. O preparo químico-mecânico consistiu da instrumentação com limas K e hipoclorito de sódio 1,0% e na irrigação final, o ácido cítrico a 6,0% foi utilizado para remoção da smear layer. Os condutos foram obturados com o cimento de OZE e todos os dentes restaurados com compósito fotopolimerizável. A avaliação clínica e/ou radiográfica ocorreu no primeiro mês e sucessivamente durante seis meses até cinco anos pós-tratamento ou até a erupção do sucessor permanente.Resultados: O índice de sucesso observado foi de 91,5%. Após cinco anos de acompanhamento, trinta e oito dentes permanentes sucessores (80,9%) estavam erupcionados. Destes, cinco (10,6%) elementos apresentaram erupção ectópica e cinco (10,6%) apresentaram opacidades no esmalte. Não foi observada associação entre o desfecho do tratamento (sucesso ou insucesso) e determinadas variáveis observadas, como o motivo da alteração pulpar, diagnóstico pulpar, presença de lesão periapical e extensão da obturação. Conclusão: Após cinco anos de acompanhamento, as pulpectomias de dentes decíduos anteriores com remoção de smear layer apresentaram alto índice de sucesso sugerindo que o uso do ácido cítrico favoreceu o desempenho da terapia. Foi observado um número reduzido de opacidades do esmalte e de erupção ectópica nos permanentes sucessores.


A partir dos resultados observados, recomenda-se a realização de novos estudos clínicos, preferencialmente controlados e randomizados, utilizando o mesmo protocolo e incluindo molares, a fim de comprovar a efetividade da remoção de smear layer nas pulpectomias de dentes decíduos


Objective: To evaluate the efficacy de pulpectomies with removal of smear layer in primary anterior teeth and the clinical conditions of the permanent successors after five years of clinical and radiographic follow up.Method: Clinically healthy children aged 3 to 5 years requiring root canal treatment in anterior teeth due to caries and/or dentoalveolar trauma were selected. The chemomechanical preparation consisted of instrumentation with K files and 1.0% sodium hypochlorite and final irrigation with 6.0% citric acid for removal of smear layer. The canalswere filled with ZOE and all teeth were restored with photoactivated composite resin. Clinical and/or radiographic evaluation was carried out in the first month and then successively during six months up to five years posttreatment or up to eruption of the permanent successor.Results: The success rate was 91.5%. After five years of follow up, 38 permanent successors (80.9%) had erupted. From these, 5 (10.6%) teeth erupted ectopically and five (10.6%) presented enamel opacities. There was no association between treatment outcome (success or failure) and certain factors, such as pulpal alteration, pulpal diagnosis, presence of periapical lesion and apical limit of obturation. Conclusion: After five years of follow up, the pulpectomies in primary anterior teeth with removal of smear layer presented a high success rate, suggesting that the use of citric acid was favorable to the therapy. There was a small number of enamel opacities and ectopic eruptions of the permanent successors. Based on the obtained results, it is recommended the development of new clinical studies, especially controlled and randomized trials, using the same protocol and including molars, in order to confirm the effectiveness of smear layer removal in the pulpectomies of primary teeth

ABSTRACT

The mechanical control of supragingival biofilm is accepted as one of the most important measures to treat and prevent dental caries and periodontal diseases. Nevertheless, maintaining dental surfaces biofilm-free is not an easy task. In this regard, chemical agents, mainly in the form of mouthwashes, have been studied to help overcome the difficulties involved in the mechanical control of biofilm. The aim of this paper was to discuss proposals for the teaching of supragingival chemical control (SCC) in order to improve dentists' knowledge regarding this clinical issue. Firstly, the literature regarding the efficacy of antiseptics is presented, clearly showing that chemical agents are clinically effective in the reduction of biofilm and gingival inflammation when used as adjuvant agents to mechanical control. Thus, it is suggested that the content related to SCC be included in the curricular grid of dental schools. Secondly, some essential topics are recommended to be included in the teaching of SCC as follows: skills and competencies expected of a graduate dentist regarding SCC; how to include this content in the curricular grid; teaching-learning tools and techniques to be employed; and program content.

Introdução: Objetivou-se avaliar a dimensão educativa das percepções e atitudes de médicos, enfermeiros e auxiliares de enfermagem na saúde bucal de crianças e adolescentes portadores de insuficiência renal crônica (IRC) em três hospitais do Rio de Janeiro. Métodos: Realizou-se uma entrevista com perguntas abertas e fechadas com 43 profissionais de saúde. os dados foram coletados e tabulados no programa SPSS 13.0. Empregou-se o teste Qui-quadrado, com nível de significância estatística p<0,05. Resultados: A média de idade foi 36,5 anos (+-11,3), com 80% do sexo feminino. A maioria dos médicos (71,4%, n=10) e enfermeiros (72,4%, n=21) acredita que esses pacientes podem ter alguma alteração bucal decorrente da doença, sendo perda de esmalte e descalcificação as mais citadas. Em relação à orientação sobre higiene bucal, pouco mais da metade da amostra respondeu que orienta seus pacientes no sentido de realizar escovação (72,7%, n=16), usar fio dental (9%, n=2), fazer bochecho (18,1%, n=4) e limpar a língua (9%, n=2). Apenas 9% (n=2) orientam a higiene após o uso de medicamentos. Quanto à necessidade de cuidados diferenciados para esses pacientes, 65,5% dos enfermeiros e auxiliares de enfermagem acreditam que estes devam ocorrer. Para a maioria dos médicos (57,1%, n=8), não há necessidade de tais cuidados. Apesar disso, 78,6% (n=11) dos médicos têm o hábito de encaminhar os pacientes a serviços odontológicos. Conclusões: Diante da metodologia empregada, concluiu-se que a maioria dos profissionais de saúde têm algum conhecimento sobre saúde bucal, porém suas atitudes não refletem esse fato.


Introduction: This study aimed to evaluate the educational dimension of perceptions and attitudes of doctors, nurses and nursing assistants in oral health of children and adolescents with chronic renal failure (CRF) in three hospitals in Rio de Janeiro. Methods: We conducted an interview with open questions and closed with 43 health professionals. data were collected and tabulated in SPSS 13.0. We applied the Chi-square test with significance level p <0.05. Results: The mean age was 36.5 years (+ -11.3), with 80% female. Most physicians (71.4%, n = 10) and nurses (72.4%, n = 21) believed that these patients may have some changes due to oral disease, and loss of enamel decalcification and the most cited. In relation to guidance on oral hygiene, just over half the sample responded that guides his patients to hold toothbrush (72.7%, n = 16), floss (9%, n = 2), make mouthwash ( 18.1%, n = 4) and clean the tongue (9%, n = 2). Only 9% (n = 2) underlying the hygiene after using drugs. The need special care for these patients, 65.5% of nurses and nursing assistants believe that they must occur. For most physicians (57.1%, n = 8), there is no need for such care. Nevertheless, 78.6% (n = 11) of doctors are accustomed to refer patients to dental services. Conclusions: Given the methodology, it was concluded that most health professionals have some knowledge about oral health, but their attitudes do not reflect this fact.

ABSTRACT

Objetivo: Avaliar o efeito citotóxico de diferentes concentrações de hipoclorito de sódio sobre uma cultura de células de osteoblastos humanos (linhagem HOB). Métodos: Culturas confluentes de osteoblastos humanos (linhagem HOB) foram obtidas em meio de Dulbecco modificado, suplementado com 10% de soro fetal bovino e submetidas a incubações com hipoclorito de sódio (concentrações de 0,5; 1,0; 2,5 e 5,25%) durante trinta segundos. O grupo controle foi representado por células incubadas em fosfato de potássio. A avaliação da viabilidade celular foi realizada através do teste de exclusão com azul de Trypan, em triplicata. Durante o período de incubação, imagens foram registradas através de um microscópio óptico invertido, para avaliação da morfologia celular. Resultados: Verificou-se que no grupo controle havia 98,7% de células viáveis, morfologicamente normais, enquanto que nos grupos experimentais, células viáveis não foram observadas. A cinética de citotoxidade seguiu tendência dependente da concentração. Conclusão: O hipoclorito de sódio nas concentrações 0,5; 1,0; 2,5; 5,25%, incubado por trinta segundos em cultura de osteoblastos humanos é citotóxico.


Objective: To evaluate the cytotoxic effect of different amounts of sodium hypochlorite, on a culture of human osteoblastos (HOB) cells. Method: Cultures of human osteoblasts (HOB) in Dulbecco's modified Eagle's medium (DMEM) supplemented with 10% of bovine fetal serum were incubated in sodium hypochlorite (concentrations of 0.5; 1.0; 2.5 and 5.25%) for thirty seconds. The control group was represented by cells incubated in phosphate buffered saline (PBS). Cell viability was assessed by means of 0.4% trypan blue dye exclusion test, in triplicate. During the incubation period, images were recorded through an inverted optic microscope to evaluate the cellular morphology. Results: In the control group 98.7% of viable cells were verified, without morphology alterations, while no viable cells were observed in the experimental groups. The kinetics of cytotoxity was concentration-dependent. Conclusion: It was concluded that there was a cytotoxic effect on cultures of human osteoblasts incubated for thirty seconds in sodium hypochlorite in all concentrations (0.5; 1.0; 2.5 and 5.25%).

ABSTRACT

The objective of the present article is to report an uncommon case of a boy with history of 11 natal lost teeth, all belonging to the normal series of primary dentition, associated with absence of up to 21 permanent tooth germs. Such a condition resulted in the necessity for oral rehabilitation. Partial removable prostheses were used as a choice of treatment, and the patient will have to be constantly followed up so that new treatment approaches can be performed according to the patient’s development.


O objetivo do presente artigo é reportar um caso incomum de um paciente infantil com história de 11 dentes natais extraídos, todos da série normal da dentição decídua, além de ausência de 21 germes da dentição permanente. Essa condição tornou necessária a reabilitação protética deste paciente. O tratamento de escolha foi a utilização de próteses parciais removíveis, e o paciente terá que ser constantemente acompanhado para que, conforme seu crescimento, novas alternativas de tratamento sejam utilizadas.

ABSTRACT

Diversas são as técnicas de tratamento endodôntico em dentes decíduos, as quais variam, principalmente, quanto às substâncias utilizadas na desinfecção dos canais radiculares e pastas obturadoras. A literatura odontológica sobre este assunto é ampla e complexa, apresentando diferentes conclusões quanto à melhor modalidade de terapia pulpar para a dentição decídua. Assim, o objetivo foi revisar a literatura odontológica sobre a variabilidade de diferentes técnicas de tratamento endodôntico em dentes decíduos e discutir os pontos que influenciam no êxito deste tratamento.


They are many techniques for pulpectomy in primary teeth, which vary, mainly, according to the substances used in the disinfection of root canals and filling pastes. Dental literature on this subject is ample and complex, presenting different conclusions regarding the best modality of pulp therapy for primary teeth. Thus, the aim of this paper is to review the dental literature on the variability of different techniques of pulpectomy in primary teeth and to argue the points that influence the success of this treatment.
